C&C provides information about, and support for, Death With Dignity (DWD) to patients, physicians, and other medical providers. C&C's medical director is available to answer physicians' questions about the law. For physicians whose patients are enrolled in our Client Service Program, C&C can help coordinate the DWD process and can provide the information required to complete the Attending Physician's After Death Reporting Form.

We recommend that every patient who is considering DWD be referred directly to C&C to ensure that the patient has ongoing support throughout the process, including presence at the time of death, if requested.
